Polycystic Ovary Syndrome (PCOS): A common cause of irregular periods and irregular ovulation. Women with PCOS often have difficulty producing mature eggs due to hormonal imbalance.

Clomiphene works by blocking oestrogen receptors in the brain, causing the release of gonadotropins, which in turn stimulate the ovaries. This medication is usually taken for 5 days at the beginning of the woman’s cycle and always at the lowest effective dose.

2. Letrozole

Letrozole is an aromatase inhibitor and is also used for ovulation induction. Although initially used for the treatment of breast cancer, it has proved effective in inducing ovulation, so much so that in recent years it has become a first-line medication for ovulation induction. Letrozole works by reducing estrogen levels, which stimulates the release of gonadotropins from the pituitary gland. This medicine is also taken as a tablet and is usually taken for 5 days at the beginning of the woman’s cycle and always at the lowest effective dose.

3. Gonadotropins

Gonadotropins are injectable medications that contain hormones such as FSH (follicle-stimulating hormone) and LH (luteinizing hormone). These hormones enhance the development and maturation of follicles in the ovaries. Gonadotrophins are usually reserved for women who do not respond to clomiphene or letrozole because they are more invasive and more expensive.

Procedure and Monitoring

The ovulation induction process begins with taking the appropriate medication at the beginning of the menstrual cycle. The dose and type of medication vary depending on the individual and diagnosis. As mentioned above, the goal is to achieve the release, ideally, of one egg using the lowest effective dose of the ovulation-inducing medication.

During treatment, the Fertility/Reproductive Medicine specialist will closely monitor follicle development through ultrasound examinations and blood tests to measure hormone levels. Once the follicles reach the appropriate size, an injectable dose of HCG (human chorionic gonadotropin) may be administered to induce final maturation and egg release.

The monitoring process is important to ensure that the ovaries do not overreact to the medications, which can lead to complications such as Ovarian Hyperstimulation Syndrome (OHSS) and multiple pregnancies (e.g., twins, triplets, etc.).

Possible Side Effects and Risks

Although ovulation induction is generally safe, there are certain side effects and risks that must be taken into account:

Multiple Pregnancies: The risk of twins or higher-order multiple pregnancy increases, which can lead to complications during pregnancy, such as preterm delivery and low birth weight.

Ovarian Hyperstimulation Syndrome (OHSS): A rare but serious condition where the ovaries react excessively to the medications, leading to pain, swelling, and fluid accumulation in the abdominal cavity and chest.

Side effects from medications: Patients may experience symptoms such as hot flushes, headaches, nausea, breast tenderness and mood changes.

Emotional Impact: The process can cause anxiety and emotional stress, which is important to address with support from the doctor and the family and friends. The process of addressing subfertility can be psychologically demanding, and emotional support is critical.
